Last revised September 10, 2026.

Respect and equal access

First Choice’s commitment is to treat people respectfully and provide access without unlawful discrimination.

Ask about language or communication assistance

Call 571-325-2340 or email [email protected]. Tell the team your preferred language or communication format and the kind of help you need. The eligibility and professional-contact forms also ask for preferred language.

Ask the team how qualified interpretation, translated information, auxiliary aids, and alternative formats can be arranged, including any assistance that must be provided at no cost. Translated versions of this website are not currently available.

Raise an access concern

Use the phone number or email above to request the responsible person for an access or discrimination concern. Please do not send health records by ordinary email.

You can also read the U.S. Department of Health and Human Services guidance on filing a civil-rights complaint. The applicable process depends on the organization and complaint.
